Military Device Protection plan is available at the time you purchase your phone, so don’t delay.
Coverage includes loss, theft, accidental damage from handling (including liquids), mechanical or electrical malfunction (after the expiration of the manufacturer’s warranty), and where available, loss and theft coverage.

With the Program, you are eligible for unlimited claims for screen break, accidental damage from handling and mechanical or electrical malfunction (after the expiration of the manufacturer’ warranty). You are also eligible for two loss or theft claims in any rolling 12-month period. Each claim is subject to a maximum value of $1,999.99, depending on your device tier.

How much does it cost?
Subscribers enrolled in the Military Device Protection Program pay a low Monthly Fee with prices ranging from $5 to $16 depending on the device’s retail MSRP at time of enrollment and the protection chosen. How do I file a claim?
How many Claims am I allowed?
The insurance policy for loss and theft allows up to 2 claims in any rolling 12-month period. The service contract for accidental damage and malfunction allows unlimited claims for screen break, accidental damage from handling and warranty malfunction. Each claim is subject to a maximum value of $1,999.99, depending on your device tier.
What do I do if I have issues uploading my documentation to My Plan?
If you are experiencing issues uploading your claim affidavit or other requested documents, you can email them to documents@Likewize.com or you can send by mail to Likewize Device Protection, LLC Attn: 1900 W Kirkwood Blvd Suite 1600C Southlake, TX 76092. Requested documentation sent by mail or email may be delayed.
Will I get the same phone for an approved claim?
Your replacement phone will be one of like kind and quality. While reconditioned equipment functions just like new, it may have minor cosmetic flaws and contain non-original manufacturer parts and accessories. If the exact make and model is not available, your replacement will be of like kind and quality. Replacement devices come with a 90-day warranty. In the event that the replacement device fails to function properly simply contact Likewize to resolve the issue.
What do I do if my phone is lost or stolen?
If your claim is for a lost or stolen phone, report the loss or theft to Likewize at 1-877-625-4893 as soon as possible so they can suspend your service, as you will be responsible for any further network charges.
How does unlimited claims for screen break repairs and accidental damage work?
With Military Device Protection, you are eligible for unlimited claims for screen break repair at a $29 deductible each as well as unlimited claims for all other damage repair at the specified amount based on device tier.
When do I file a Claim?
If your phone or device is lost, stolen or damaged file a claim.
How long do I have to file a claim?
A claim for loss and theft must be filed within 60 days of the incident and proof of loss may be required. All other claims can be filed at any time as long as you are enrolled in the program.
If my device is replaced or repaired, am I protected for the new device?
Replacement phones come with a 90-day warranty. In the event the device malfunctions during this 90-day warranty period, we will repair or replace it at no cost to you. Repairs come with a 90-warranty on the repair performed. If you experience issues with the repair during this time, we will repair or replace your phone at no cost to you.
If you are having issues with your device during this period, please follow the existing claim process to report the problem.
Will I get the same device for an approved claim if my phone is lost, stolen, or unrepairable?
Your device will be replaced with a wireless device of like kind and quality with comparable features and functionality and may be new or reconditioned.
Am I required to pay a deductible before I get my device repaired or replaced?
Yes. You are required to pay a non-refundable deductible for each approved claim.
How will I be billed for my monthly fee?
Your first monthly fee will be charged at the time of purchase and prorated for the remainder of the month. Your full monthly fee will then be charged to the credit card used to enroll on the first of each subsequent month.
How do I cancel coverage?
You may cancel your coverage at any time by contacting Likewize at 1-877-625-4893.
What should I do with my damaged device?
If you receive a replacement device due to an approved accidental damage from handling or malfunction claim, you’ll need to return your broken device to Likewize. Shipping instructions and shipping label for returning the damaged equipment will be included with the replacement device. Be sure to act quickly! Failure to do so within 30 days will result in a non-returned equipment fee up to the MSRP of your device at the time of enrollment.
What determines which repair options are offered?
Your location, device model, type of damage, and repair or replacement part availability can influence your repair choices. For example, brand new models may not be available for repair right away, and certain types of damage such as liquid damage are not eligible for repair.
Can I go to any phone repair location?
To ensure quality, all repairs must be completed through an authorized repair partner provided and chosen at the time you file your claim.
What if there’s not a repair location near me, but I don’t want to replace my device?
Don’t worry, we have repair partners who accept devices via mail. Likewize will provide a prepaid label. Just package your device and drop it in the appropriate mailbox. In most cases, it will be repaired within 3-5 business days from receipt and returned to you via next business day delivery.
What happens if the repair provider cannot fix my device?
If your device is unrepairable, you will receive a replacement.
What happens if I have an issue with my device after repair?
Repaired devices come with a 90-day warranty for the repair made. If you have an issue with the repair during this time, we will repair or replace your device at no cost to you. Just follow the existing claim process to report the problem.

The Military Device Protection program is a repair and replacement service provided to The Mobile Center customers. The Military Device Protection program is a combination of an Insurance Policy and a Service Contract. The Insurance Policy covers loss and theft. In the state of Washington, the loss and theft insurance policy is underwritten by Zurich American Insurance Company. In all other states the loss and theft insurance policy is underwritten by Universal Underwriters Insurance Company. Likewize Agency, LLC is the licensed agent and Likewize Device Protection, LLC administers the program. In California, the Agent is Likewize Agency, LLC d/b/a Likewize Insurance Agency, LLC. The Service Contract covers Accidental Damage from Handling and Mechanical and Electrical Failure. Unless stated otherwise, the Provider of the Plan is Likewize Device Protection, LLC, 1900 W.
